The Compass Program aims to support patients with mild cognitive impairment to late-stage dementia.
For patients, we utilize a four-color identification system based on the Global Deterioration Scale©, we create an individualized care plan for each patient that provides quality of life, encourages meaningful social interactions, and promotes resident safety and function.
Powerback clinicians utilize the Brief Cognitive Assessment Tool (BCAT) for early identification and intervention related memory,
problem solving, and safety impairments.
With each individualized care plan, The Powerback Compass Program can suggest environmental modifications to facilitate patient independence and engagement.
Providing opportunities to
participate in activities of interest lessens anxiety and behaviors while promoting socialization.
The Compass Program focuses on person-centered facility education, training modules, and approaches to care.
The goal of this training is simple: to promote consistent, positive interactions with cognitive care patients, while minimizing negative and/or unsafe behaviors.
The Powerback Compass Program is designed to provide ongoing support to your existing facility programming.
A 2 point change in the BCAT from admission to discharge supports increased cognition and
improved performance for hygiene, dressing, bathing, self-feeding.
Powerback’s average improvement is +3.3 points for patients with a Cognitive Care focus.
A Flexible Endoscopic Evaluation of Swallowing (FEES) is a test conducted by a Speech-Language Pathologist that determines how efficiently a patient can swallow.
A flexible fiberoptic endoscope is used to examine a patient's throat and swallowing capacity in order to best recommend which foods and drinks are safest for consumption.
This test is more sensitive to diagnosing dysphagia (difficulty swallowing) than traditional bedside swallowing examinations, and can be re-administered as needed: reducing altered diet and feeding guideline survey deficiencies.
With Powerback, a FEES is conducted on-site: allowing patients to stay within their community to undergo this 10-15 minute procedure without the need to consider transporation costs, or in-patient hospital visits.
Did you know? Following a consult, FEES results from Powerback are presented immediately, leading to enhanced resident satisfaction and quick treatment targeting. Reporting is uploaded to an EMR in less than one business day, and includes high-definition color imaging.
With Powerback's FEES Swallow Solutions Program, cut down scheduling time to less than a week between referral and diagnostic testing.
This combination of faster scheduling, in-house testing, accurate diagnosis, and immediate results can improve quality of life while promoting aging in place.
Our FEES Service is available to centers who have partnered with Powerback, or, as an individual service. Even if you utilize another preferred rehab provider in your care center, we're here to improve your patient outcomes, and help your center gain a marketing advantage.
Swallow Solutions' FEES identify the need for additional treatment or potential further diet changes in over 90% of cases*, as well as immediate diet changes in 60%* of individuals.
